This article explains how to access and read a client’s active support plan in Lumary DC, on both the desktop interface and the mobile app. When a support plan is activated for a client, it becomes available for all workers supporting them to review. Workers can view the plan’s sections and items, submit a compliance declaration to confirm they have read and understood the plan, and — on mobile — capture observations, restrictive practice usage and behavioural episodes directly from the plan during a shift.

Viewing an active support plan
The quickest way to access a client’s active support plan is from the Support Plan component at the top of their record.
- Go to the client’s record.
- Click the arrow on the Support Plan component at the top of the page. The active support plan opens.

Viewing all support plans
- Go to the client’s record.
- Select the Support Needs tab, then select Risks, Assessments & Plans. Scroll down to the Support Plans section.
- Click on a Support Plan ID to open the record. The plan displays all sections and items, with details drawn from the client’s record.
Viewing an audit trail of historical changes
- From the support plan record, click the Related tab.
- Scroll down to the Support Plan History section. This shows a log of all changes made to the plan, including the date, field changed, user, original value, and new value.
Adding a declaration on desktop
If you are yet to to submit a compliance declaration confirming you have read and understood the plan, you will notice a blue notification at the top of the page.
- Click the arrow icon on the Support Plan notification at the top of the page (or click on a Support Plan ID to open the full Support Plan record)
- Scroll to the bottom of the plan and click Add Declaration.
- Verify the pre-filled declaration details are correct.
- Click Save. The Add Declaration button will be hidden for your user once the declaration is submitted, to prevent duplicates. Compliance records are automatically created for the support plan and its linked assessments and plans.
Viewing a support plan on mobile
Support workers can access a client’s active support plan during a session. When the client’s record is opened from the session, the plan is displayed directly on the record as a mobile-optimised card layout, making it easy to scan and review relevant items on the go.

Open your session/shift from the home screen or calendar.

Find the client in the Site Clients list and tap their name to open their record.

The client’s active support plan displays directly on the record as a list of expandable cards, organised into sections.

Tap a card title or the chevron icon to expand an item and view its full details. Items with a high risk rating or high-priority client contact display a HIGH badge.
Adding a declaration on mobile
- Scroll to the bottom of the support plan and tap Add Declaration.

Verify the pre-filled details and tap Save.
