Lumary DC - This article explains how to create a goal attainment plan.
By setting and monitoring goals, you can show how your support and services help clients improve their lives. Using Lumary, you can create goal attainment plans and track your client's progress. You can use them to track NDIS Plan goals, as well as other service goals.
To create a goal attainment plan:
- Navigate to the client's record.
- Click on the Goals tab.
- Click on the Goal Plans tab.
You can also create goal attainment plans from a client's onboarding process. This may depend on your business's onboarding processes.
- Click on the Add a new Goal Attainment Plan button.
- This will launch the New Goal Attainment Plan form. Enter the details for your new plan.
The following table explains each of the fields on the New Goal Attainment Plan form. Mandatory fields are marked with an asterisk. Some fields displayed in your environment may be different, depending on how it has been set up for your business processes.

| * Client | The system will auto-populate this field with the client's name. |
| * Plan Start Date | The date this plan will begin. Defaults to today's date. |
| * Plan End Date | The date this plan will end. |
| Comments | Any comments that will help describe the plan. |
| * Plan Review Status | How the plan is progressing. Not usually required when setting up a new plan. |
- Once you've filled in all the fields on the form, click on the Save button.
This will take you to the new Goal Attainment Plan record. You can now start adding NDIS Plan goals and agreed service goals to the plan.
