Lumary DC - This article introduces goal setting. It provides an overview of goal attainment plans, and the connection to NDIS plan goals.
Lumary enables providers to set up and monitor goals, so that they can show how their support and services help clients improve their lives. These goals can be based on the goals in a client's NDIS Plan or goals defined separately with the client.
Active plans and goals appear under the Goals tab on the client's record, where users can see:
- goals grouped by priority level
- icons that represent different types of goals
- actions and strategies
- the estimated completion date
- a link to a related NDIS Goal, if applicable
- the professionals responsible for helping the client to achieve the goal
- the goal's current status
- the category of the goal, and
- how many days are left until the estimated completion date.

Get started with goal setting
To get started, you'll need to add a goal attainment plan to a client's record.
Once you've added a plan, you can:
- capture goals from the client's NDIS plan, and
- add and monitor agreed service goals to track the client's progress, and
- attach files to the plan.
How goals relate to goal attainment plans
A Goal Attainment Plan is a structured approach used to support NDIS participants in working toward their personal goals. It connects the broader NDIS Plan Goals with practical, measurable agreed service goals developed in collaboration with service providers. This plan helps ensure that everyone involved understands what the participant wants to achieve and how support services will contribute to these outcomes.
During day-to-day service delivery, workers can capture ongoing progress tracking against their agreed service goals.
As an NDIS plan period comes to a close, conducting a thorough End of Plan Review is essential to ensure that the supports provided have been effective in helping the participant work towards their goals. During an End of Plan review, Recommendations can be captured about support that will help a participant achieve their goals.

Goal tracking within the Salesforce mobile app
Workers logged into Lumary DC using the Salesforce mobile app are able to view a client's goal attainment plans and access attached files. Workers are not currently able to view details of the NDIS goals or agreed service goals that make up the plan.
