Lumary DC - This article explains how to list risks to a client, staff or others and display them as alerts on a client's record.
The system allows you to capture a range of risks that could affect how you provide services to a client. Critical risks can also be displayed as alerts to help notify workers before they provide services.
Alerts can also be displayed for health conditions, high intensity needs, and personal support plans.
To add a risk:
- Go to the client's record.
- Click on the Support Needs tab.
- Click on the Risks, Assessments & Plans tab.
- Scroll down to the Client Risks section.
- Click on the New button.
This will open a New Client Risk form. The following table describes the fields in this form. Mandatory fields are marked with an asterisk. Filling in some fields reveals or activates additional options.

| * Client | This field is auto-filled with the name of the client. |
| Status | Set the status to Active. |
| Display as Alert | Put a tick in the checkbox to add the risk to the alerts displayed on the client's record. |
| Alert Summary |
Appears if you tick the Display as Alert checkbox. You can provide a description of the risk, which will be appended after the domain (Domain: Alert Summary) when the risk is displayed as an alert. |
| Enquiry | Use this field to search for a related enquiry record if required. Enter the client's name to list their related enquiries. You may need the enquiry record number. |
| Does this Client Risk constitute a High Intensity Need? | A tick in this checkbox will launch a New High Intensity Need form when you click on the Next button at the end of this form. |

| Domain | Use the drop-down list to select a domain. |
| Date identified | Use the calendar to select the date the risk was identified. |
| Sub Domain |
Selecting a domain activates the Sub Domain field. The options will depend on which domain you selected. You can select more than one sub domain:

Initial Assessment The next part of the form covers the risk assessment, including consequence, likelihood and mitigation actions. |

- Click on the Next button.
If you've indicated that the risk constitutes a high intensity need, you'll need to complete a New High Intensity Need form.
- Once you complete any additional forms that appear based on your selections, review the details.
- Click on the Finish button.
The new record will now be listed under Client Risks on the Support Needs tab. To edit or delete this record:
- Click on the drop-down arrow at the end of the row.
- Select Edit or Delete.
If you selected Display as Alert, the risk will be listed as an alert at the top of the client's record. This will prompt workers to consider this risk when they interact with the client.
