Lumary DC – This article explains how to record a request for a client’s personal information.
Personal information request records can be used to track:
- the request type
- who asked for the information
- key dates
- documents provided, and
- the authority to release
Keeping these details in one place helps your organisation meet compliance obligations.
To record a personal information request:
- Go to the client’s record.
- Click on the Preferences & Legal tab.
- In the Personal Information Requests section, click on the New button.
This will open a New Personal Information Request form. The following table describes the fields in this form. Mandatory fields are marked with an asterisk.

| Date received | Use the calendar picker to record the date the request was received |

Select the request category. For example, options may include:

| Who requested information | Free-text field to note the person or organisation that made the request. |
| Date responded to | Use the calendar picker to enter the date your organisation responded to the request. |
| Description of documentation | Free-text field where you can summarise the documentation provided in response to the request. |
| * Client | This field is filled with the client’s name. |
| Status |
Track the progress of the request. For example:

| Date completed | Use the calendar picker to set the date the request was finalised. |
| Amount raised | Numerical field to capture any charges raised in handling the request. |
| File types included |
Show the types of files provided. For example:

| Authority to release by | Search for and select the worker who authorised the release. |
| Personal information request ID | Read-only identifier generated after the record is created. |
- Click Save to create the request.
The request will appear in the Personal Information Requests section on the client’s Preferences & Legal tab.
To edit or delete this record:
- Click the drop-down arrow at the end of the row.
- Select Edit or Delete.
